I've had my san pedro for just over a year, but he's starting to look all distorted and horrible, I don't -think- I'm underwatering him.

At what size did you buy it? It could be still used to it's previous perfect atmosphere.
At my last workplace we had a bunch of them, the older ones slowly started to grow this way aswell.

Have you checked to see if it is rooted?

Cactus cuttings can survive on their own for an insanely long time. Years even. Check the roots. Also, what kind of soil is it in? Sandy I hope.

Not enough sun and not enough nutrients. Look into getting some cactus nutrients and possibly repotting it in a larger container with a good mixture of sand/perlite and cactus soil.

It might also help to water it from underneath by putting it in a dish of shallow water for a little while. This forces the cactus to grow its roots towards the water and gives it a firmer hold in the soil.

What kind of soil are you using?
